The overuse of and reliance on checklists is related to this failure to eliminate basic design flaws. Checklists are often constructed using information from previous accidents. Although checklists are useful in learning from the past and making sure that obvious things are not forgotten, blindly using checklists alone can also limit the factors that are considered.
